Fermilab Weekly Safety Report Archive – 2015

Each week, Fermilab Today will publish the safety report compiled by the Fermilab ESH&Q section. It includes information on injuries reported at the Medical Office as well as the overall safety performance at the lab.


July 7, 2015
This week's safety report, compiled by the Fermilab ESH&Q Section, contains one incident. An employee was stung by a wasp on his right arm while throwing something into a dumpster. He received first-aid treatment.

June 23, 2015
This week's safety report, compiled by the Fermilab ESH&Q Section, contains two incidents. An employee reported a spider bite to the Medical Office after his arm became puffy and tender. This case is report-only. An employee returning from travel experienced neck and back pain after sitting on a plane for seven hours. While retrieving her luggage, she experienced more back pain. This is a pending claim.

June 9, 2015
This week's safety report, compiled by the Fermilab ESH&Q Section, contains two incidents. An employee stepped off a one-foot ledge, lost his balance and fell. He caught himself on his left arm. He reported to medical the following day with shoulder pain and was later seen by an orthopedist. X-rays for fracture were negative. Medication was prescribed, making this case recordable. A contract employee cut a finger on his right hand while pushing a cart of metal sheeting spools. He received first-aid treatment.

June 2, 2015
This week's safety report, compiled by the Fermilab ESH&Q Section, contains one incident. An employee received a superficial laceration on his thumb while using a screwdriver to pry a digital pressure meter from its chassis. The screwdriver slipped, and the employee's thumb contacted the side of the chassis. He received first-aid treatment.

This week's safety report, compiled by the Fermilab ESH&Q Section, contains one incident. An employee noticed bumps on his arm while at work. He received first-aid treatment.

May 12, 2015
This week's safety report, compiled by the Fermilab ESH&Q Section, contains two incidents. An employee was injured while calibrating a high-pressure relief valve. The valve was propelled with great force, struck the ceiling and fell, contacting the employee on the back of the head. The employee suffered a contusion and a laceration requiring four sutures. This is a recordable injury and an ORPS. An employee has a confirmed standard threshold shift. An investigation is under way. This is a pending claim.

This week's safety report, compiled by the Fermilab ESH&Q Section, contains one incident. An employee working in an awkward position felt a pain in his left side where it had been pressed against a cabinet. He received first-aid treatment.

This week's safety report, compiled by the Fermilab ESH&Q Section, contains one incident. An employee missed the bottom step while descending a step ladder. He fell, striking a desk chair on the way down, and fractured a rib. This is a DART case and will be entered into the DOE Occurrence Reporting and Processing System.

This week's safety report, compiled by the Fermilab ESH&Q Section, contains one incident. An employee tripped and fell when his boot caught on an imperfection in the parking lot at Site 38. He received first-aid treatment.

March 24, 2015
This week's safety report, compiled by the Fermilab ESH&Q Section, contains one incident. An employee sustained an abrasion on his left knee after slipping in the parking lot. It was snowing heavily at the time. The employee received first-aid treatment.

March 17, 2015
This week's safety report, compiled by the Fermilab ESH&Q Section, contains two incidents. An employee strained her back while attempting to prevent a child from sitting on another child. She was pulled off balance and while trying to catch herself twisted her back. This is a DART case. A visitor fell in the parking lot when her ankle gave out. Her computer bag struck her middle right finger in the fall. The visitor went to the Medical Office and was advised to go to urgent care for X-rays. This is a pending claim.

March 10, 2015
This week's safety report, compiled by the Fermilab ESH&Q Section, contains two incidents. An employee cut his hand on a sharp protrusion on a piece of equipment. He received first-aid treatment. An employee slipped in a parking lot during freezing rain. As he attempted to prevent himself from falling he wrenched his back, left hip and knee. He received first-aid treatment. Salt was applied after the slip was reported.

March 3, 2015
This week's safety report, compiled by the Fermilab ESH&Q Section, contains two incidents. An employee fell on the sidewalk and dislocated her right shoulder and scraped on her right knee. An employee fell off a ladder and injured his right ankle. This is a pending claim.

This week's safety report, compiled by the Fermilab ESH&Q Section, contains one incident. An employee confirmed hearing loss in both ears. This is a pending claim.

Feb. 17, 2015
This week's safety report, compiled by the Fermilab ESH&Q Section, contains one incident. A employee felt pain in his lower back. He was prescribed medicine and is attending physical therapy.

She received first-aid treatment. A magnet power supply experienced an electrical safety interlock error.
