Fermilab Weekly Safety Report — 2008 Archive

Each week, Fermilab Today will publish the safety report compiled by the Fermilab ES&H section. It includes information on injuries reported at the Medical Department as well as the overall safety performance at the lab.

Tuesday, Dec. 16
This week's safety report, compiled by the Fermilab ES&H section, includes no injuries. We have now worked 42 days since the last recordable injury.

This week's safety report, compiled by the Fermilab ES&H section, includes no injuries. We have now worked 35 days since the last recordable injury.

This week's safety report, compiled by the Fermilab ES&H section, includes no injuries. We have now worked 28 days since the last recordable injury.

Tuesday, Nov. 25
This week's safety report, compiled by the Fermilab ES&H section, includes one injury, which was not reportable. We have now worked 21 days since the last recordable injury.

Tuesday, Nov. 11
This week's safety report, compiled by the Fermilab ES&H section, includes three injuries, one of which is reportable. An employee pinched a finger in the hinge of a vanity being tossed into the trash.

Tuesday, Oct. 28

This week's safety report, compiled by the Fermilab ES&H section, lists no reportable injuries. We have now worked 18 days since the last recordable injury.

Tuesday, Oct. 21

This week's safety report, compiled by the Fermilab ES&H section, lists three injuries reported last week. One injury, in which an employee's leg was pinned between a die table and a motorized jack, is reportable. We have now worked 11 days since the last recordable injury.

Tuesday, Oct. 7

This week's safety report, compiled by the Fermilab ES&H section, lists three injuries reported last week. One case required medical treatment and will also be a DART case. The other two injuries were minor and required little or no treatment. The TRC Rate for FY08 was 0.66 and the DART Rate was 0.22. These rates are less than half of what they were a year ago. Congratulations to all!

Tuesday, Sept. 30

This week's safety report, compiled by the Fermilab ES&H section, lists three injuries reported last week. Two were minor first-aid injuries and the third is still under investigation. The injury rate calculations appearing in this report are still estimates until final end-of-quarter "hours worked" are reported by our subcontractors during the next few days.

Tuesday, Sept. 23

This week's safety report, compiled by the Fermilab ES&H section, lists two incidents. The laboratory has worked 34 days without a reportable accident.

Tuesday, Sept. 16

This week's safety report, compiled by the Fermilab ES&H section, lists two minor first-aid incidents. The laboratory has worked 27 days without a reportable incident.

Tuesday, Sept. 9

This week's safety report, compiled by the Fermilab ES&H section, lists two minor incidents, neither one recordable. We have now worked 20 days since the last reportable incident.

Tuesday, Sept. 2

This week's safety report, compiled by the Fermilab ES&H section, lists no incidents. We have now worked 13 days since the last recordable incident.

Tuesday, Aug. 26

This week's safety report, compiled by the Fermilab ES&H section, lists two incidents. One was a minor first-aid injury caused by a person tripping up the Auditorium stairs. An employee also fractured a finger when replacing the belts on a fan. This incident is a recordable case and involves restricted work activity (DART case). Tuesday, Aug. 19

This week's safety report, compiled by the Fermilab ES&H section, includes a reportable incident. An employee suffered a second-degree burn on the right forearm when the arm touched a hot exhaust pipe as the person tried to turn off a valve. Also, a student hurt a toe when the person tripped walking up the auditorium stairs. The case is pending.

Tuesday, Aug. 12

This week's safety report, compiled by the Fermilab ES&H section, lists two incidents. ES&H is reviewing whether the second incident was work-related.

Tuesday, Aug. 5

This week's safety report, compiled by the Fermilab ES&H section, lists four minor first-aid cases, none of which are recordable. Fermilab employees and contractors have worked 100 days without a recordable injury. The last page of the safety report summarizes the number of DART and TRC cases for employees and subcontractors since 1997 and shows the great progress we have made in reducing the number of injuries incurred by our employees and contractors.

This week's safety report, compiled by the Fermilab ES&H section, lists no reportable injuries. There were two minor first aid cases last week. We have now worked 93 days since our last recordable case, approximately 1 million hours.

This week's safety report, compiled by the Fermilab ES&H section, lists no reportable injuries.There were four minor injuries during the past week but none of them required medical treatment beyond first aid. We have now worked 86 days without a recordable injury. We have worked 210 days since our last DART case. Congratulations to all for this ES&H achievement!

This week's safety report, compiled by the Fermilab ES&H section, lists no reportable injuries. The laboratory has worked 79 days without a reportable injury.

This week's safety report, compiled by the Fermilab ES&H section, lists no reportable injuries. One injury from April was reclassified as a reportable injury. Since that incident, the laboratory has worked 72 days without a reportable injury.

This week's safety report, compiled by the Fermilab ES&H section, lists two minor first-aid injuries, both non-recordable. Fermilab has not had a reportable injury for almost three months.

This week's safety report, compiled by the Fermilab ES&H section, lists two minor first-aid injuries, both non-recordable. The last recordable injury for a Fermilab employee or contractor occurred 80 days ago.

This week's safety report, compiled by the Fermilab ES&H section, shows that a minor injury in February 2008 was upgraded from non-reportable to reportable. This increases the total number of reportable cases in FY08 to seven. There were no injuries reported last week. Fermilab has worked 21 days without recordable injury.

Tuesday, March 11
This week's safety report, compiled by the Fermilab ES&H section, includes two cases. One reportable, first-aid case included an individual who needed first aid to remove a staple from between their finger and fingernail. A non-reportable case involved a person injuring their back while lifting.

Tripping and slipping incidents are among the most frequent incidents reported at Fermilab, especially during winter time. This week's safety report, compiled by the Fermilab ES&H section, includes two recordable injuries resulting from slipping on ice. The laboratory now has five recordable cases for the fiscal year.

Tuesday, January 29
This week's safety report, compiled by the Fermilab ES&H section, provides an update on a Dec. 22 accident in which a subcontractor fell on ice. The accident counts as a DART case (Days Away, Restricted or Transferred) since the subcontractor lost days of work. Fermilab now has two DART cases and four recordable cases for the fiscal year.
