Fermilab Weekly Safety Report Archive – 2012

Each week, Fermilab Today will publish the safety report compiled by the Fermilab ES&H section. It includes information on injuries reported at the Medical Department as well as the overall safety performance at the lab.

Tuesday, December 18
This week's safety report, compiled by the Fermilab ESH&Q section, contains one incident. An employee received a minor laceration on his hand. He received first-aid treatment.

This week's safety report, compiled by the Fermilab ES&H section, contains two incidents. An employee suffered from tendonitis after using a hand tool. Restricted work makes this case a DART. The employee walked into a tree, striking his left eye. He received first-aid treatment.

Tuesday, November 27
This week's safety report, compiled by the Fermilab ES&H section, contains one incident. An employee suffered a minor laceration to his finger when his hand accidentally brushed up against sharp metal. He received first-aid treatment.

Tuesday, November 20
This week's safety report, compiled by the Fermilab ES&H section, contains two incidents. An employee suffered minor abrasions and bruising after falling down three steps. She received first-aid treatment. An employee noted back pain while moving rocks and concrete. This case is currently under investigation and categorized as a pending claim.

An employee has a confirmed standard threshold shift in his left ear. This case is claim pending and still under investigation.

This week's safety report, compiled by the Fermilab ES&H section, contains two incidents. An employee suffered a contusion when the hoist he was working with fell over. Medical treatment makes this case recordable. An employee backed a government vehicle into a private vehicle causing minor damage. There were no injuries.

This week's safety report, compiled by the Fermilab ES&H section, contains one incident. An employee suffered a hematoma to his finger while moving a magnet. He received first-aid treatment.

Tuesday, October 16
An employee tripped and fell in the parking lot. She received first-aid treatment. A subcontract employee was struck in the arm by a saw, resulting in a laceration. Sutures make this case recordable (FY12). Another subcontract employee was struck on the elbow by the handle of a hammer, resulting in a contusion. He received first-aid treatment (FY12). An employee was stung by a bee. He received first-aid treatment. An employee has a confirmed standard threshold shift in his left ear. This case is pending claim and still under investigation.

Tuesday, October 09
This week's safety report, compiled by the Fermilab ES&H section, contains one incident. While opening a chemical container, an employee was splashed in the eye with ethyl alcohol and potassium hydroxide. She received first-aid treatment.

Tuesday, October 02
This week's safety report, compiled by the Fermilab ES&H section, contains two incidents. An employee suffered an elbow strain while closing a valve. Limited duty work makes this case a DART. A piece of stainless steel weighing less than 10 pounds fell from above and hit an employee on his welding helmet. He was not injured.

This week's safety report, compiled by the Fermilab ES&H section, contains two incidents. A contract employee suffered a back strain when he lifted a garbage can full of garbage and water. Medical treatment and lost time make this case a DART. An employee experienced a minor skin rash when his wrist made contact with the dirty surface of a cable tray. He received first-aid treatment.

He received first-aid treatment. Medical treatment and lost time make this case a DART. An employee aggravated a pre-existing back injury while moving a cart. Additional medical treatment and lost days will accrue against the first report of injury. An employee suffered a minor avulsion on his finger while moving a relay rack. He received first-aid treatment. Several fiberglass slivers came off in an employee's hand while he was holding a fiberglass pole. He received first-aid treatment. An employee suffered a minor puncture wound while using a scribe tool. She received first-aid treatment. An employee suffered a minor laceration when his hand struck the edge of a folder. He received first-aid treatment.

Tuesday, August 21
This week's safety report, compiled by the Fermilab ES&H section, contains two incidents.Two employees were stung by wasps. They received first-aid treatment.

Tuesday, August 14
This week's safety report, compiled by the Fermilab ES&H section, contains two incidents. A day-care teacher strained her shoulder while handling a baby. She received first-aid treatment. An employee noticed a tick on her neck while working at her computer. She received first-aid treatment.

Tuesday, August 07
This week's safety report, compiled by the Fermilab ES&H section, contains four incidents. A contract employee suffered a hernia while pulling cables. Medical treatment plus days away from work make this case a DART. An employee suffered a minor injury to her eye when she poked it with a file folder. She received first-aid treatment. An employee was stung by a wasp and received first-aid treatment. An employee reported that he removed a tick that was crawling on his leg.

Tuesday, July 31
This week's safety report, compiled by the Fermilab ES&H section, contains five incidents. One employee tripped, slipped and fell, hitting his head on an elevator door. He suffered a laceration resulting in medical treatment and one lost work day. The case is a DART. Three separate employees were stung by wasps. They received first-aid treatment. An employee suffered a minor laceration when the window he was holding broke. He received first-aid treatment.

Tuesday, July 24
This week's safety report, compiled by the Fermilab ES&H section, contains six incidents. One employee was stung by a wasp. Another employee was stung by a wasp and suffered a subsequent infection. Medical treatment and lost time make this case a DART. Another employee was stung by a bee and suffered a subsequent infection. Medical treatment makes this case recordable. One employee turned and stumbled, falling with both arms outstretched to break his fall, and consequently fractured both wrists. He required medical treatment and lost time, making this case a DART. An employee was struck in the eyebrow by a brass coupling while removing a transfer line. Medical treatment makes this case recordable. An employee suffered minor irritation on her finger while scraping paint. The case required first-aid treatment.

Tuesday, July 17
This week's safety report, compiled by the Fermilab ES&H section, contains two incidents. Two employees and one visitor were stung by wasps. They required first-aid treatment. An employee was injured when the rug she stepped on slipped out from underneath her, causing her to fall. Medical treatment and lost time make this case a DART.

Tuesday, July 10
This week's safety report, compiled by the Fermilab ES&H section, contains five incidents. Two subcontract employees had ticks embedded in their skin. They removed the ticks and reported to the Medical Department as a precaution. An employee suffered a minor laceration when he struck his arm against a fixed object. He received first-aid treatment. Another employee was stung by a wasp. The employee received first aid treatment. Another employee was struck on the lower left lip when the lifting strap he was pulling on came loose. He received first-aid treatment. A contractor employee in AD had been working in the Main Injector enclosure for a period of 8 weeks without wearing a personnel dosimetry badge. He had been wearing a pocket dosimeter and was tabulating his dose. AD is currently investigating to determine the errors and organizational weaknesses that led to this unwanted outcome.

Tuesday, July 02
This week's safety report, compiled by the Fermilab ES&H section, contains five incidents. A subcontractor working off-site slipped off a platform, injuring his hand and thumb and abrading his back and knee. This is a DART case but is not a recordable case since the contracting company has fewer than 10 employees. An employee noted an insect bite on his/her arm. An employee was stung by a bee while surveying a dumpster of scrap metal. The case is not recordable. Another employee was stung twice by a wasp. He received first-aid treatment only. An employee reported to the medical office for itching and rash, possibly arising from contact with poison ivy.

This week's safety report, compiled by the Fermilab ES&H section, contains three incidents. An employee received first-aid treatment after suffering minor abrasions after falling off his bike. Another employee strained his back while moving a pallet. Medical treatment makes this case recordable. Another employee received first-aid treatment after his knee popped as he entered a work vehicle.

This week's safety report, compiled by the Fermilab ES&H section, contains one incident. An employee developed back pain from having to work in an awkward and tight position and was diagnosed with back strain. The case is a DART because of the day away from work.

Tuesday, May 08
This week's safety report, compiled by the Fermilab ES&H section, contains one incident. A contractor employee fell down a flight of stairs, fracturing his ankle. The case is classifed as ORPS.

Tuesday, May 01
This week's safety report, compiled by the Fermilab ES&H section, contains three incidents. An employee had a severe contusion to his hand after it was caught between a chain sling and an angle plate on a mill table. The case is a DART. Another employee experienced back pain after stopping suddenly while driving a heavy load. He received first-aid treatment. Another employee was doused with sprinkler water while working at Argonne. Treatment was not needed.

Tuesday, Apr. 24
This week's safety report, compiled by the Fermilab ES&H section, contains two recordable incidents. An employee had a back strain after taking water samples from several different wells over a two day period. He received medical treatment and significant work restrictions, making this case a DART. Another employee experienced wrist pain after working on her laptop all day. She received medical treatment, and the case is recordable.

Tuesday, Apr. 17
This week's safety report, compiled by the Fermilab ES&H section, contains two incidents. A contract employee was injured after falling down the stairs. He received medical treatment and had days away from work, making this case a DART. There was also a management concern after a component was removed without authorization at the Linac Klystron Station 8. AD is following up on the incident, including corrective and preventative actions.

A third employee was diagnosed with a cumulative trauma resulting in medical treatment, making this case is recordable.
