Fermilab Weekly Safety Report Archive – 2011

Each week, Fermilab Today will publish the safety report compiled by the Fermilab ES&H section. It includes information on injuries reported at the Medical Department as well as the overall safety performance at the lab.

Tuesday, Dec. 20
This week's safety report, compiled by the Fermilab ES&H section, includes no incidents or injuries.

Tuesday, Dec. 13
This week's safety report, compiled by the Fermilab ES&H section, includes one recordable injury and one incident. An employee strained his back while picking up a box, and he lost work time as a result of the injury. Unrelated to the injury, a natural gas line in the Village was obscured by a tree root and cut during an excavation to repair and replace sections of the sanitary sewer. The incident was not categorized as ORPS, as a lessons learned document is being generated.

Tuesday, Dec. 06
This week's safety report, compiled by the Fermilab ES&H section, includes three injuries, one of which was recordable. An employee fell out of a van he was unloading, and he lost work time as a result of the injury, classifying the incident as DART. Another employee pinched his finger while transporting a pipe. He was wearing gloves, but required first-aid treatment. A third employee was hit in the face by an aluminum can while moving a cabinet. He received first-aid treatment.

Tuesday, Nov. 29
This week's safety report, compiled by the Fermilab ES&H section, includes one incident that was not recordable. An employee was struck by a vehicle while walking from a conference to his hotel. He was not injured and did not require first-aid treatment.

Tuesday, Nov. 22
This week's safety report, compiled by the Fermilab ES&H section, does not include any incidents.

Tuesday, Nov. 15
This week's safety report, compiled by the Fermilab ES&H section, includes one incident that was recorded as a DART case. An employee hit his head after slipping and falling in the back of a pick-up truck. The employee was transported to the hospital where he was evaluated, treated and released. Medical treatment and lost time make this case a DART.

Tuesday, Nov. 08
This week's safety report, compiled by the Fermilab ES&H section, includes one incident that was recorded as a DART case. An employee strained his back while pulling cables, resulting in medical treatment and restricted work.

Tuesday, Nov. 01
This week's safety report, compiled by the Fermilab ES&H section, includes two incidents, one of which was recordable. A subcontractor employee injured his fingers and received sutures, making the injury a recordable case. Another employee suffered an abrasion after tripping on the stairs and received first-aid treatment.

Tuesday, Oct. 25
This week's safety report, compiled by the Fermilab ES&H section, includes one incident and a reclassification of an incident last week as recordable. The incident involving an employee who stepped through a rotted wooden floor and fell was reclassified as a near-miss, recordable incident. Another employee received first-aid treatment after accidentally burning his finger with a soldering iron.

Tuesday, Oct. 18
This week's safety report, compiled by the Fermilab ES&H section, includes two incidents, neither of which were recordable. An employee stepped through a rotted wooden floor and fell. He received first-aid treatment. Another employee received first-aid treatment after getting a sliver in his thumb while tightening a brass bolt.

Tuesday, Oct. 11
This week's safety report, compiled by the Fermilab ES&H section, includes three incidents, none of which were recordable. An employee was stung twice by a wasp, and another employee struck his finger with a rubber mallet, but neither required first-aid treatment. A third employee required first-aid treatment after getting his hand caught between a crane attachment and a sign.

Tuesday, Oct. 04
This week's safety report, compiled by the Fermilab ES&H section, includes one incident. A service subcontractor bumped his finger on a car door and was seen in the Medical Office, but did not require first-aid treatment.

Tuesday, Sep. 27
This week's safety report, compiled by the Fermilab ES&H section, includes no incidents.

Tuesday, Sep. 20
This week's safety report, compiled by the Fermilab ES&H section, includes one incident. It was not recordable. An employee received first-aid treatment after dropping 25 pounds of aluminum on his foot.

Tuesday, Sep. 13
This week's safety report, compiled by the Fermilab ES&H section, includes four incidents. One was recordable. Two separate employees twisted their ankles while walking on gravel. Both required first-aid treatment. A third employee required first-aid treatment for a sore and swollen ankle after falling on the stairs in Wilson Hall. A fourth employee fractured their ankle while getting out of a work truck.

Tuesday, Sep. 06
This week's safety report, compiled by the Fermilab ES&H section, includes four incidents. One was recordable. An employee was stung by an insect, but did not require first-aid treatment. Another employee suffered a contusion on his abdomen when he was struck by the handle of a cart, and he received first-aid treatment. A third employee required first-aid treatment after he experienced back pain while lifting. A fourth employee had a foreign object in his eye that required medical treatment and prescription drugs, making the incident recordable.

Tuesday, Aug. 30
This week's safety report, compiled by the Fermilab ES&H section, includes three incidents. One incident was recordable. An employee suffered an avulsion on his finger tip while moving a dewar with a hand cart. the employee required medical treatment and lost five days of work. Another employee received a minor contusion on his head after hitting the door of a vehicle he was exiting, and he required first-aid treatment. A third employee lacerated her finger while using scissors to snip wire, and she received first-aid treatment.

Tuesday, Aug. 23
This week's safety report, compiled by the Fermilab ES&H section, includes two incidents. A service subcontractor felt back pain after using a small hand grinder while bending over an RF cavity. He received first-aid treatment. A guest scientist, traveling out of the country on business, was bitten by a dog and required medical treatment.

Tuesday, Aug. 16
This week's safety report, compiled by the Fermilab ES&H section, includes three incidents. An employee was stung by an insect, but did not require first-aid treatment. Another employee required first-aid treatment after handling a fiberglass rod and finding a foreign object in his hand. Another employee required first-aid treatment after he was swarmed by wasps while on a ladder. He was stung and injured his knee while jumping down.

Tuesday, Aug. 09
This week's safety report, compiled by the Fermilab ES&H section, includes four incidents requiring first-aid treatment. An employee recieved a minor injury to his face when he was struck by a steel towing bar on a car. Another employee was struck on the left side of the forehead while unrolling flex hose. Two separate employees were stung by insects.

Tuesday, Aug. 02
This week's safety report, compiled by the Fermilab ES&H section, includes one incident requiring first-aid treatment. A visitor twisted his knee while stepping off of the FNAL shuttle bus.

Tuesday, July 26
This week's safety report, compiled by the Fermilab ES&H section, includes one recordable incident, two incidents requiring first-aid treatment and two additional incidents. One employee received a laceration on his finger when he caught his hand between two objects while moving a lifting fixture. He required sutures to close the laceration. Two separate employees were stung by wasps and required first aid treatment. One employee tripped and fell, resulting in a disloacted finger. He set the finger himself and did not require first aid treatment. Another incident involved a vehicle accident, but no one was injured.

Tuesday, July 19
This week's safety report, compiled by the Fermilab ES&H section, includes one recordable incident, one injury requiring first-aid treatment and one incident not requiring first-aid treatment. One employee received a laceration on his thumb while moving a lifting fixture that required sutures to close. Another employee received an abrasion required first-aid treatment to the side of his finger while dismantling a pneumatic valve. Another employee hit a car while parking his or her vehicle, but no one was injured.

Tuesday, July 12
This week's safety report, compiled by the Fermilab ES&H section, includes no recordable incidents and three injuries requiring first-aid treatment: One employee received minor lacerations on his hands after he fell while rushing into Wilson Hall, and two other employees were injured while trying to enter Wilson Hall during Monday's storm. One was blown over by a gust of wind, and another turned her ankle on a curb.

Tuesday, July 5
This week's safety report, compiled by the Fermilab ES&H section, includes no recordable incidents and two injuries requiring first-aid treatment: One employee had a burr in his or her left index finger and another was stung by a wasp.

Tuesday, June 28
This week's safety report, compiled by the Fermilab ES&H section, includes no recordable incidents and two injuries requiring first-aid treatment: An employee tripped on carpeted stairs, twisted an ankle and ended up with a broken fibula and displaced bones of the ankle, and an employee bumped the right rear bumper of a parked car with the front bumper of employee’s car.

Tuesday, June 22
This week's safety report, compiled by the Fermilab ES&H section, includes no recordable incidents and three injuries requiring first-aid treatment: While moving equipment, a service subcontractor suffered a finger injury; An employee noticed a tick on his leg and needed medical help to have it removed; and an employee was stung by an insect.

Tuesday, June 14
This week's safety report, compiled by the Fermilab ES&H section, includes no recordable incidents and two injuries requiring first-aid treatment: An employee received a minor cut when the screwdriver he was using slipped and hit his hand, and an employee suffered a small puncture from a mechanical pencil.

Tuesday, June 07
This week's safety report, compiled by the Fermilab ES&H section, includes one recordable incident: An employee suffered an abrasion and a strain when the chair he sat on fell apart.

Tuesday, May 31
This week's safety report, compiled by the Fermilab ES&H section, includes no recordable incidents and three injuries/vehicle accidents: An employee overextended her wrist while unloading paper, requiring first-aid treatment; a contractor backed his truck into FCC, shattering a window but resulting in no injuries; and an employee was stopped at the stoplight at Kirk and Wilson street when the government vehicle he was driving was struck from behind (rear ended) by another vehicle, requiring first-aid treatment.

Tuesday, May 24
This week's safety report, compiled by the Fermilab ES&H section, includes no recordable incidents and two incidents that required first-aid treatment: An employee suffered low back and side back pain after moving his things from one office to another, and an employee suffered a small cut on his right forearm, possibly from the handle on a cabinet.

Tuesday, May 17
This week's safety report, compiled by the Fermilab ES&H section, includes one recordable incident. An employee tripped and fell in the parking lot. Medical treatment makes this case recordable.

Tuesday, May 10
This week's safety report, compiled by the Fermilab ES&H section, includes one recordable incident. An employee suffered a laceration when a drill bit from the hand drill he was using came in contact with the top of his hand. Medical treatment makes this case recordable.

Tuesday, May 03
This week's safety report, compiled by the Fermilab ES&H section, includes no recordable incidents. Two first aid-only incidents occurred this week: An employee bumped her elbow on a copier and a service subcontractor fell and skinned her knee.

Tuesday, Apr. 26
This week's safety report, compiled by the Fermilab ES&H section, includes one recordable incident: An employee suffered a small, second degree burn when the hot oil the employee was handling splashed onto his or her wrist.

Tuesday, Apr. 19
This week's safety report, compiled by the Fermilab ES&H section, includes two recordable incidents: an employee lacerated a finger on a metal cabinet and another employee suffered a broken finger when hit by the door of a portable toilet blown open by wind.

Tuesday, Apr. 12
This week's safety report, compiled by the Fermilab ES&H section, includes two incidents. An employee bumped his or her head on a piece of unistrut and another was hit in the foot by a crowbar that fell out of a vehicle.

Tuesday, Apr. 5
This week's safety report, compiled by the Fermilab ES&H section, lists no recordable incidents. An employee had elbow pain after removing a pump from a pit and another employee cut a fingertip while stripping wire. Both required first-aid treatment only.

Tuesday, Mar. 29
This week's safety report, compiled by the Fermilab ES&H section, lists no recordable incidents. An employee hurt their ankle while walking, but was not seriously injured.

Tuesday, Mar. 22
This week's safety report, compiled by the Fermilab ES&H section lists no recordable incidents. A guest scientist tripped and fell at the airport while on business travel. First aid treatment.

Tuesday, Mar. 15
This week's safety report, compiled by the Fermilab ES&H section, lists two recordable incidents. A piece of debris became stuck in an employee's eye while he was using a sander. A subcontracter slipped on black ice and struck her head. She was treated at a hospital.

Tuesday, Mar. 08
This week's safety report, compiled by the Fermilab ES&H section lists no recordable incidents. An employee slipped on the property and reported to medical, but no treatment was necessary.

Tuesday, Mar. 01
This week's safety report, compiled by the Fermilab ES&H section lists no recordable incidents. An individual tripped on a piece of raised concrete in front of Wilson Hall but was not injured. FESS is investigationg solutions to the raised concrete problem.

Tuesday, Feb. 22
This week's safety report, compiled by the Fermilab ES&H section lists no recordable incidents. One employee slipped and fell on partially thawed ice in the parking lot. The employee was not hurt, but reported to Medical anyway.

Tuesday, Feb. 15
This week's safety report, compiled by the Fermilab ES&H section lists one recordable incident.An employee slipped and fell on ice in the parking lot, injuring his shoulder. The injury resulted in medical treatment.

Tuesday, Feb. 08
This week's safety report, compiled by the Fermilab ES&H section lists no recordable incidents. Two employees slipped on snow-covered ice in parking areas.

Tuesday, Feb. 01
This week's safety report, compiled by the Fermilab ES&H section lists no reportable incidents.

Tuesday, Jan. 25
This week's safety report, compiled by the Fermilab ES&H section lists no reportable injuries. A truck hit a building after sliding on ice.

Tuesday, Jan. 18
This week's safety report, compiled by the Fermilab ES&H section, includes three incidents, one of which was a reportable injury. The two first-aid only injuries included a weather-related fall and an employee who tripped over uneven ground. The reportable injury involved an employee who cut his hand on a piece of equipment.
