Fermilab Weekly Safety Report Archive – 2014

Each week, Fermilab Today will publish the safety report compiled by the Fermilab ES&H section. It includes information on injuries reported at the Medical Department as well as the overall safety performance at the lab.

Tuesday, December 16
This week's safety report, compiled by the Fermilab ESH&Q Section, contains two incidents. An employee's finger became caught between a door and a latch, resulting in a contusion. The employee was given ice and a bandage. An employee reported to the Medical Office that he experienced difficulty hearing.

Tuesday, December 9
This week's safety report, compiled by the Fermilab ESH&Q Section, contains one incident. An employee fainted, causing him to fall and injure his right ankle. X-rays showed that his ankle was fractured.

Tuesday, December 2
This week's safety report, compiled by the Fermilab ESH&Q Section, contains one incident. The chlorine level upstream of national pollutant discharge elimination system outfall 002 measured 0.29 milligrams/liter; the permitted limit is 0.05 mg/L. The Fermi Site Office and Fermilab notified the Illinois EPA within 24 hours of discovery. Since the system was shut down, chlorine levels at the outfall dropped to 0.04 mg/L.

Tuesday, November 25
This week's safety report, compiled by the Fermilab ESH&Q Section, contains three incidents. An employee sprained his shoulder while lifting heavy objects. He returned to work with limitations on the use of his left arm. An employee lacerated his ear while emerging from above a ceiling. He received medical treatment. A variable-frequency drive for a fan blew apart, also blowing off the panel door, which hit an employee in the torso. He received medical attention and was released with no restrictions.

Tuesday, November 18
This week's safety report, compiled by the Fermilab ESH&Q Section, contains two incidents. A subcontractor fell, hitting his head and shoulder. He was treated and released by emergency room staff. This is a DART case. An employee experienced shoulder pain. He was examined by urgent care.

Tuesday, November 4
This week's safety report, compiled by the Fermilab ESH&Q Section, contains one incident. A contractor developed red, raised bumps after wearing powder-free gloves for three days off and on. He was given a prescription.

Tuesday, October 28
This week's safety report, compiled by the Fermilab ESH&Q Section, contains one incident. Molten material from a weld landed on some foam board insulation, causing a fire that burned through about three square feet of foam board. There were no injuries.

Tuesday, October 21
This week's safety report, compiled by the Fermilab ESH&Q Section, contains one incident. A subcontractor was flushing resin from a tank when a drop of water splashed into his eye, causing a burning sensation. He was wearing safety glasses. His eye was rinsed at the Medical Office.

Tuesday, October 14
This week's safety report, compiled by the Fermilab ESH&Q Section, contains three incidents. An employee reported feeling discomfort in his shoulder and has continued to feel aches since reporting it. An employee pinched his finger on an actuator. He received first-aid treatment. An employee struck his head on a board of wood. The wound was cleaned and bandaged.

This week's safety report, compiled by the Fermilab ESH&Q Section, contains one incident. An employee reported feeling discomfort in his left shoulder on Sept. 23. He has continued to experience aches since and has been given restrictions keeping him from performing his normal jobs functions. Restrictions are being accommodated.

This week's safety report, compiled by the Fermilab ESH&Q Section, contains two incidents. An employee caught his hand between a material lift device and some cabinets. He received medical treatment. This is a DART case. An employee noticed a redness on his skin, which itched. He received first-aid treatment.

Tuesday, September 23
This week's safety report, compiled by the Fermilab ESH&Q Section, contains six incidents. An employee was stung by a bee. He received first-aid treatment. An employee's finger was crushed between two large cylinders. He received medical treatment. An employee was stung by a yellowjacket. He received first-aid treatment. An employee inhaled fumes from overheating batteries on a forklift, causing excessive coughing. He left the building immediately and went to the Fermilab medical office. An employee's hand struck a cabinet in her work area, resulting in injury. She received medical treatment. An employee is experiencing lower back and right side pain. This is a pending claim.

Tuesday, September 16
This week's safety report, compiled by the Fermilab ESH&Q Section, contains two incidents. An employee tripped over a raised area of concrete, falling and injuring her hand, knee and chin. She received sutures. An employee was stung by a bee. He received first-aid treatment.

This week's safety report, compiled by the Fermilab ESH&Q Section, contains one incident. An employee felt pain in his lower back shortly after lifting a box. This is a DART case.

This week's safety report, compiled by the Fermilab ESH&Q Section, contains one incidens. A contract employee received a cut from a shim between the fingers of his left hand. He received sutures. This is not a DART case.

Tuesday, July 29
This week's safety report, compiled by the Fermilab ESH&Q Section, contains two incidents. An employee tripped over some fencing lying on the ground and heard a snap in his foot. He was sent off site for X-rays. An employee developed blisters, which developed into ulcers, after wearing steel-toe shoes for three or four days. He was sent off site for treatment.

Tuesday, July 22
This week's safety report, compiled by the Fermilab ESH&Q Section, contains one incident. An employee received a yellowjacket sting on his shin, which immediately swelled. He was given some restrictions and a walking boot, and he returned to work the next day.

This week's safety report, compiled by the Fermilab ESH&Q Section, contains one incident. While breaking a beaver dam, an employee grabbed a sharp branch and received an abrasion on his right hand. He received first-aid treatment.

This week's safety report, compiled by the Fermilab ESH&Q Section, contains two incidents. An employee's eye became irritated. He flushed his eye with water. An employee felt acute back pain while lifting roughly 10 pounds. He was sent to the emergency room.

Tuesday, June 3
This week's safety report, compiled by the Fermilab ESH&Q Section, contains three incidents. Debris flew into an employee's eye while He was changing a filter, resulting in a corneal abrasion. He was prescribed antiobiotics. A contractor neglected to put his truck into park and ran through the roll-up door at Lab 4. While unwrapping an accelerator cavity shipped here from another laboratory, Fermilab employee noticed the smell of gas. He immediately wrapped it back up. The incident is being investigated.

Tuesday, April 29
This week's safety report, compiled by the Fermilab ESH&Q Section, contains three incidents. A subcontrator fractured the tip of one finger when it was caught between a pipe wrench handle and the frame of a pipe threading machine. This is a DART case. An employee was hurt when a nearby cart carrying roughly 50 pounds of material tipped over, dumping the load onto his back. This is a pending claim. An employee felt sudden pain in one finger, part of turned a bluish color. This is a pending claim.

This week's safety report, compiled by the Fermilab ESH&Q Section, contains two incidents. An employee was struck in the face when a strong wind pushed on the door she was opening. She received first-aid treatment. An employee hurt his right wrist and hand when his hand was caught in a door. Cold compresses and elevation were recommended.

She received first-aid treatment. A supervisor accidentally entered the NuMI pre-target and target hall without the correct keys. He self-reported the controlled-access violation.

Tuesday, February 4
This week's safety report, compiled by the Fermilab ESH&Q Section, contains three incidences. An employee tripped, hitting his face against a cabinet and receiving a laceration. He received first-aid treatment. A small piece of metal fell into and was embedded in an employee's eye while he was welding. He was wearing a face shield. The employee received medical treatment. This is a DART case. An employee struck his head against a light fixture, resulting in a small abrasion. He received first-aid treatment.

Tuesday, January 28
This week's safety report, compiled by the Fermilab ESH&Q Section, contains one incident. A scientist emeritus and an employee were making a controlled access to work on some electronics. They had pulled the electronics and some cables. Some cables were strung across the floor in the aisle in front of the electronics racks. As the SE turned to walk in another direction, his foot caught on a cable, causing him to lose his balance and fall. He hit his forehead on the concrete floor, resulting in a laceration. He was transported to the hospital and received 5 sutures. Recordable injury.

Tuesday, January 21
This week's safety report, compiled by the Fermilab ESH&Q Section, contains one incident. On Jan. 13, Fermilab exceeded the NPDES-permitted chlorine limit at Kress Creek. The laboratory reported the excess to the Illinois Environmental Protection Agency, shut down the bleach feed into the industrial cooling water system, contacted divers to inspect the integrity of the piping and delivery systems, and ordered a series of chemical analyses.

Tuesday, January 14
This week's safety report, compiled by the Fermilab ESH&Q Section, contains three incidents. An employee slipped and twisted his ankle while walking down some stairs. He received first-aid treatment. An employee slipped in the parking lot. This case is report-only. An employee slipped on black ice on her way to her car. This case is report-only.

Tuesday, January 07
This week's safety report, compiled by the Fermilab ESH&Q Section, covers the last three weeks and contains five incidents. While securing a bag of bolts to a steel plate, an employee lost his grip on the plate. The plate fell, pinching the employee's fingers between it and the floor, causing a laceration. While exiting truck in the parking lot, an employee's foot slid out from underneath him causing him to fall to the ground on his back. Some spilled lubricant fell on top of an employee's safety glasses and into his right eye. He flushed his eye with water for 15 minutes. A small piece of insulation broke off and blew into a contract employee's mouth while he was affixing a batt to a window, causing him to choke. He eventually caught his breath. He received first-aid treatment. An employee pulled a cart out of his vehicle's path and struck his finger on the edge of a nearby dumpster, causing nail avulsion and laceration. He is scheduled for surgery.
