Fermilab Weekly Safety Report — 2009 Archive

Each week, Fermilab Today will publish the safety report compiled by the Fermilab ES&H section. It includes information on injuries reported at the Medical Department as well as the overall safety performance at the lab.

Tuesday, December 29
This week's safety report, compiled by the Fermilab ES&H section, includes one, non-recordable injury involving an employee who slipped on icy pavement. We have now worked 14 days since the last recordable injury.

Tuesday, December 22
This week's safety report, compiled by the Fermilab ES&H section, includes one recordable injury. We have now worked seven days since the last recordable injury.

Tuesday, December 15
This week's safety report, compiled by the Fermilab ES&H section, includes four non-recordable injuries, including one involving reaching into a stuck drawer. We have now worked 54 days since the last recordable injury.

Tuesday, December 8
This week's safety report, compiled by the Fermilab ES&H section, includes one non-recordable injury. The injury occurred when a contractor pulled his calf muscle while pushing a spool of wire. We have now worked 47 days since the last recordable injury.

This week's safety report, compiled by the Fermilab ES&H section, includes no injuries. We have now worked 40 days since the last recordable injury.

Tuesday, November 17
This week's safety report, compiled by the Fermilab ES&H section, includes two first-aid injuries. We have now worked 26 days since the last recordable injury.

Tuesday, November 10
This week's safety report, compiled by the Fermilab ES&H section, includes no injuries. We have now worked 19 days since the last recordable injury.

This week's safety report, compiled by the Fermilab ES&H section, includes no injuries. We have now worked 12 days since the last recordable injury.

Tuesday, October 27
This week's safety report, compiled by the Fermilab ES&H section, includes two first aid cases and one recordable injury case. An employee was carrying a 25-pound cart up three flights of stairs with a coworker and noticed back pain the next day. The medical office gave the employee prescription medication, making it a recordable injury.

Tuesday, October 20
There was one first aid case reported to the Medical Department this week.

Tuesday, October 13
There were no injuries reported this week.

Tuesday, October 6
There was one non-recordable injury reported this week. A subcontractor sprained an ankle by stepping on the hose of a fume extractor.

Tuesday, September 29
This week's safety report, compiled by the Fermilab ES&H Section, includes one first aid and one recordable injury case. The recordable injury was a laceration an employee received while cutting aluminum on a table saw. The piece of aluminum kicked back and caused the injury. The investigation revealed that the blade used was not appropriate for cutting metal. We have now worked three days since the last recordable injury.

This week's safety report, compiled by the Fermilab ES&H section, includes no reportable accidents. We have now worked 18 days since the last recordable injury.

This week's safety report, compiled by the Fermilab ES&H section, includes no reportable accidents. We have now worked 11 days since the last recordable injury.

Tuesday, August 25
This week's safety report, compiled by the Fermilab ES&H section, includes five incidents reported to the Medical Office last week. Two were first-aid cases and three were recordable incidents. Two of the recordable cases involved wasp stings that required prescription medication. The third was a standard threshold shift in an employee's hearing.

Tuesday, August 18
This week's safety report, compiled by the Fermilab ES&H section, includes five incidents reported to the Medical Office last week. One incident was a back strain that resulted in missed work (DART), one head laceration that required sutures (TRC) and three first aid cases.

Tuesday, August 11
This week's safety report, compiled by the Fermilab ES&H section, includes five incidents, including one near miss and one recordable case. The recordable injury involved a subcontractor, who felt muscle pain in their back when unloading a heavy bag of garbage. We have now worked three days without a recordable injury.

Tuesday, August 4
This week's safety report, compiled by the Fermilab ES&H section, includes one recordable case. One was a DART case involving a back strain. We have now worked six days without a recordable injury.

Tuesday, July 28
This week's safety report, compiled by the Fermilab ES&H section, includes two first aid cases. One was a reportable injury to a subcontractor who hit a burr with an angle grinder. The grinder then jumped off the steel plate and caused a 1" laceration in the person's left knee requiring six sutures. We have now worked five days without a recordable injury.

Tuesday, July 21
This week's safety report, compiled by the Fermilab ES&H section, includes three reported injuries. We have now worked 24 days without a recordable injury.

Tuesday, July 14
This week's safety report, compiled by the Fermilab ES&H section, includes no reported injuries. We have now worked 17 days without a recordable injury.

Tuesday, July 7
This week's safety report, compiled by the Fermilab ES&H section, includes 14 incidents reported in the last two weeks that were work related. Nine cases required first aid treatment or no treatment; two cases resulted in medical treatment (TRC); one case required medical treatment and days away from work (DART); and two were vehicle accidents without injury.

This week's safety report, compiled by the Fermilab ES&H section, lists one injury reported to the Medical Office that required first aid treatment. It has been 17 days since the last recordable injury.

Tuesday, June 9
This week's safety report, compiled by the Fermilab ES&H section, lists one injury reported to the Medical Office that occurred while the person was outside of the United States. It has been 10 days since the last recordable injury.

Tuesday, June 2
This week's safety report, compiled by the Fermilab ES&H section, lists three first-aid-only injuries. A fourth is under investigation. It has been 41 days since the last recordable injury.

This week's safety report, compiled by the Fermilab ES&H section, lists one reported first-aid-only injury. It has been 34 days since the last recordable injury.

Tuesday, May 19
This week's safety report, compiled by the Fermilab ES&H section, lists no reported injuries . It has been 27 days since the last recordable injury.

Tuesday, May 12
This week's safety report, compiled by the Fermilab ES&H section, lists one case reported to the Medical Department last week. An employee received a small laceration after brushing the left side of the head on a rack-shelf support. This case did not result in time away from work.

Tuesday, May 05
This week's safety report, compiled by the Fermilab ES&H section, lists two injuries reported to the Medical Department last week. An employee's hand slipped while loosening a bolt on a motor, injuring the employee's thumb. Only first aid was required. The other incident was due to a repetitive strain injury and is under investigation.

Tuesday, April 28
This week's safety report, compiled by the Fermilab ES&H section, lists one case reported to the Medical Department last week. An employee strained a shoulder while lifting 50-pound fan elements to change HEPA filters. This case resulted in time away from work.

Tuesday, April 21
This week's safety report, compiled by the Fermilab ES&H section, lists one ergonomic case reported to the Medical Department last week.

Tuesday, April 14
This week's safety report, compiled by the Fermilab ES&H section, lists four injuries reported to the Medical Department last week. Two of them were for evaluation only, the third required first-aid treatment for a spider bite and the fourth was an audiogram for hearing loss.

Tuesday, April 7
This week's safety report, compiled by the Fermilab ES&H section, lists five injuries reported to the Medical Office last week. Three of the five were minor and required only first aid treatment. A fourth case was an injury caused by a goose attack. This injury was serious enough to require time away from work. The fifth case was a head wound that required medical treatment.

This week's safety report, compiled by the Fermilab ES&H section, There was one first aid injury (slip on ice) and one vehicle accident in the last week. We have now worked 23 days since the last recordable injury.

Tuesday, Feb. 17
This week's safety report, compiled by the Fermilab ES&H section, includes two injuries that were reported to the Medical Department last week. One, a reportable incident, involved a helium contractor who fell off the back of a truck and lost consciousness. The other incident was not reportable. We have now worked 15 days since the last recordable injury.

Tuesday, Feb. 10
This week's safety report, compiled by the Fermilab ES&H section, includes There were two injuries reported to the Medical Department last week. One back strain that resulted in time away from work (DART) and one first aid injury. We have worked 11 days since the last recordable injury.

Tuesday, Feb. 03
This week's safety report, compiled by the Fermilab ES&H section, includes three incidents, one first-aid case and two recordable cases. The first recordable case involved an employee who suffered a laceration serious enough to require six sutures. The second reportable injury was to a service subcontractor who fell while trying to access a vehicle and was rendered unconsious. We have worked four days since the last recordable injury.

Tuesday, Jan. 27
This week's safety report, compiled by the Fermilab ES&H section, includes four incidents, three of which are related to snow removal. No incident resulted in a reportable injury, but one case is pending. We have worked 19 days since the last recordable injury.

Tuesday, Jan. 20
This week's safety report, compiled by the Fermilab ES&H section, includes no reportable injuries. We have now worked 12 days since the last recordable injury.

Tuesday, Jan. 13
This week's safety report, compiled by the Fermilab ES&H section, includes four injuries, two of which were reportable. Three of the four injuries were caused by employees falling after slipping on icy pavement. We have now worked five days since the last recordable injury.

This week's safety report, compiled by the Fermilab ES&H section, includes no injuries. We have now worked 20 days since the last recordable injury.
