New DOE Requirements Mean New Badges for ALL Fermilab Users

Reporting Changes in Visa Status

March 12, 2003

To: Fermilab's Non-U.S. ID Badge Holders
From: Bruce Chrisman
Subject: Reporting Changes in Visa Status

Department of Energy regulations require Fermilab to keep track of all changes in U.S. visa status for all ID badge holders, by making new entries into the DOE database.

We would like to ask your cooperation in informing Dianne Snyder in the Users Office of any change in your visa status, such as a new visa or extension, or a change from one type of visa to another.

The Users Office is located in the Wilson Hall Atrium on the first floor, west side. You may contact Dianne Snyder at extension 3811, or by e-mail at dsnyder@fnal.gov. She will be happy to answer your questions regarding these new DOE requirements.

We apologize for any inconvenience this new request may cause, and we thank you for your patience and understanding in dealing with these new conditions.
