The Survey
The issue of medical insurance affects us all. It is a complex and
undeniably crucial issue. It is especially important to those who can
least afford to pay, such as graduate students and visitors to the
lab.
Currently, Fermilab requires its Users to provide proof of valid
emergency medical insurance only.
The purpose of this survey is to gauge the level of insurance provided by their
home institutions
to members of the Fermilab User community. Are you satisfied with your
coverage? If not, how could the situation could be improved?
Your Fermilab ID number will not be used to identify you. We simply
ask for it so we can eliminate duplicate submissions of the online
form.
