You should check with your university or home institution regarding your coverage while living in Illinois. There are frequently restrictions, which apply when receiving medical care outside of the state or local area of your insurance provider.
You must have medical insurance to get a Fermilab ID, and that insurance must be in effect for the entire year your ID badge is valid. You will be asked to fill out a Certification of Medical Insurance form when applying for an ID badge. The insurance must be renewed every year in order for you to renew your Fermilab ID badge.
If you do not have medical insurance or need insurance only for the length of your stay at Fermilab, you can apply for short-term medical coverage at the Users Office, X3111, first floor Wilson Hall. The cost of this insurance, available for both individuals or families, varies from year to year; contact the Users Office for details on plans, providers, and costs.
