The first step to obtaining a Fermilab ID badge is completing the necessary
paperwork. Applications for a Visitor ID badge may be obtained from the Users
Office, or online at
http://www.fnal.gov/faw/vidform/vidform.pdf.
Page 2 of the application requires you to watch a safety video. This
video is available in the Fermilab library, or online at
http://vmsstreamer1.fnal.gov/VMS_Site_02/Training/UserSafety/index.htm
and
http://www.fnal.gov/directorate/documents/chap2.html.
Also on page 2 is a section which requires you to read up on the
Fermilab Computer Security Policy (also mentioned in Section 2.2).
These policies are located online at
http://computing.fnal.gov/cd/policy/cpolicy.pdf and
http://computing.fnal.gov/security/. Once the form is completed,
along with all the appropriate signatures, it can be faxed or mailed to
the Fermilab Users Office so it will be on file when you arrive.
It is also necessary to get into Fermilab to get your ID badge. Under the new security regulations in Section 2.1.1, you should tell the security guard you are going to Wilson Hall to get an ID, and he or she will allow you to enter. If the security level changes, then you must either be accompanied by someone with a Fermilab ID badge, or you must enter through the main gate (the Farnsworth and Kirk Rd. entrance) and get a visitor's badge. To be issued a visitor's badge, someone with a valid ID must have called security ahead of time to give them your name and purpose at Fermilab.
Once on-site, go to the Users Office in Wilson Hall to finalize the paperwork. They will then direct you to the Key and ID Office on the ground floor of Wilson Hall to have your picture taken and your ID badge printed up within minutes. At this time, all badges expire one year after they have been issued. You must complete all the same paperwork again every year to have your ID renewed. Always complete the ID renewal process before the date your current ID actually expires. If you allow your ID to expire, Fermilab will immediately freeze your Fermilab email and computer accounts.
