`Please let us know :are you a member of APS, DPF, which year of studies towards your Ph.D you are finishing, what do you plan to do after you receive your Ph.D, how long do you expect to take to finish your degree, have you attended or presented work at a previous NP, how did you find out about the New Perspectives, do you know what is the GSA at Fermilab, have you participated in any of the activities that the GSA has organized, will you came to Fermilab for the NP alone and of any other thoughts that you may have regarding the GSA and the NP.
